Course Description

You’ve learnt all about PSIRF, you understand SEIPS, you can’t remember a time when you didn’t talk about human factors on a regular basis. Now what?

Well now you need to actually organise yourself and write your learning response report. This course looks at the practical side of investigating incidents and report writing.

The one-day course will be delivered by two experienced investigators and will be case study led.

Course content

Delegates will be taken through how to organise evidence and how to write user-friendly, evidentially sound After Action Review Records, Patient Safety Incident Investigation Reports and other learning response records.

Delegates will learn

  • How to organise evidence in categories using the ‘Index’ management system
  • Methods for collecting, organising, using and storing evidence electronically
  • The importance of keeping a record of information requests
  • How to write an executive summary
  • How timelines can be used to structure reports and records
  • Ways to compile a tabular timeline in a spreadsheet
  • The advantages of using a table to present learning
  • The appropriate use of visuals
  • How to write PSII reports in plain English
  • How to use various templates (both mandatory and suggested)
  • Techniques for quality assuring their own work
