What you will learn on our Systems Approach to Investigating and Learning from Patient Safety Incidents Training

  • Understand the core purpose and principles of the Patient Safety Incident Response Framework (PSIRF)
  • Apply systems thinking and human factors methods to move beyond individual blame and understand work within its real context
  • Differentiate between PSIRF learning responses, including After Action Reviews (AARs), Debriefs, Safety Huddles and Patient Safety Incident Investigations (PSIIs)
  • Recognise how a Just and Learning Culture underpins effective patient safety work
  • Conduct effective, non-judgmental interviews and capture “Work As Done” through observations
  • Apply the duty of candour and support staff, patients, families and carers throughout the learning response
  • Use analysis and evidence-mapping tools such as the SEIPS model and the Contributory Factors Framework
  • Produce meaningful, actionable safety actions with clear measurement plans
  • Write clear, accessible reports
  • Understand the importance of monitoring the effectiveness of safety actions over time
