Systems Approach to Learning from Patient Safety Incidents Oversight Public
This training will support the development of expert understanding and oversight systems-based patient safety incident response throughout the healthcare system – in line with NHS guidance, based upon national and internationally recognized good practice.
Description
The course will contain a facilitated discussion session on changes organisations will need to make to its oversight systems and processes to be PSIRF compliant.
Those providing oversight within organisations must have an adequate level of knowledge and experience to ensure that investigations and learning responses are actively leading to improvements in patient safety.
Accordingly the session will cover:
- An overview of PSIRF and the overriding principles
- The PSIRF oversight mindset – 6 principles
- Organisational responsibility of providers of NHS funded care as well as the ICB, NHS England and the CQC
- Questions to guide provider board oversight of patient safety management and improvement
- Proportionate responses
- Discuss a just culture approach, maintaining an open, transparent and improvement focused culture
- Complex investigations spanning different organisational, care setting and stakeholder boundaries
- Effective oversight and supporting processes related to incident response including quality assurance.
- Patient safety incident investigation (PSII) commissioning and planning
